- Choose an appointment time for your new patient visit and reserve your appointment.
- Complete the patient registration form for each member of the family.
- Enter your payment information for your monthly membership fee.
- Our office will review your information and complete the registration process. Initial fees will include the monthly membership fee of $65 / member ($260 / family) and a one-time registration fee of $65 per household. Nothing is charged until your first consultation. Cancel anytime (receive a prorated return minus the registration fee).
